These forms were filled out by the same Nigerian scammer who sent all of the photography that I showed off in my LAST EGG. NOTE: I have blacked out all of his or my personal information.
If you are not familiar with what scam baiting is, then you should read THIS EGG to learn all about it.
Keep in mind that the questions and requirements in this form are outrageous and it would be very hard to get anybody to fill it out for any reason. The only reason that he filled it out for me is because he is in debt from the photography project and he still thinks that he is going to get money from me.
Once you put somebody in debt it won’t take much convincing actually

just take my money and go to the money gram and ask them to send it to my recipiant , you pay the little money that money gram will charge you , after payment they will issue you a recipt with the txt question and answer together with the control number, then you send it to me and I will call my friend obinna to go the bank for confarmations, justa little process, is it why you bring me this rubish. by the time that I sent you the 100 pics did I ask you to fill a form for me or did I ask you if you are a torrerist and communism? what has all this non-sence to do with my payments as regards the pictures I SENT TO YOU?
am,I a neative of u.s of america? then why should you asked me a question as if I am an american.let me tell you , others are sending money to their love ones with out filling this type of forms or dont you know that there are many nigerians who live in america , my own brother live in canada and I have other relation that lives in america with their wives and children.
